Drug Research Paper Inhalants are wide variety of substances that young adults use as another way to get high. Inhalants cause mind-altering effects and typically someone would not think of these products as drugs because they were never intended for that type of use. Inhalants are very easy for young adults to find and many different types are even in their own homes. Inhalants can be anything from household cleaners to aerosols to gases. Inhalants are administered by breathing in through the nose or mouth in a variety of ways sniffing or snorting the fumes from the containers.People who use them can also inhale fumes from a balloon or a bag which would be filled with the inhalant of choice. The high from the inhalant only lasts a few minutes which causes users to continue to inhale substances many times over a short period of time and having greater effects. The physical impacts caused from inhalants are very similar to what someone would be like if they were to get drunk from alco hol. Symptoms would include slurred speech, little to no coordination, euphoric, and dizziness. Users may also experience hallucinations, and delusions.Users who frequently inhale will feel less control of their body for a longer period of time and may feel drowsy and have severe headaches for several hours and can possibly carry over to days with the same headache symptoms. Depending on the chemical inhaled users will experience added effects such as confusion, nausea, increased heart rate, and throwing up. The behavioral impact from inhalants can be a short state of excitement and also confusion and hallucinations. Users also will have sudden mode swings and the inability to made clear and smart decisions.Long term effects from inhalants that are very harmful and some irreversible effect areas all over the body. One of the long-term effects that is very serious and is from excessive inhalants is the break down of myelin. Myelin is a fatty tissue that surrounds and protects nerve f ibers and helps messages get sent all over the body. Due to excessive use of inhalants will cause serious damage to the myelin and that will lead to muscle spasms, tremors, and the high possibility of losing basic abilities such as walking, bending down, and talking.Other serious irreversible long-term effects from inhalants are hearing loss, limb spasms, severe brain damage, and bone marrow damage. The use of inhalants among teens and adults is very high due to the easy access they have to wide varieties of them. Over 17 million people have experimented with inhalants. Inhalants are also the fourth most abused substance in the United States. Rohypnol is a drug that is very common and targets the central nervous system. It is used as a depressant medicine and is currently illegal for all use in the United States. Rohypnol has many names but is most commonly know as roofies or the â€Å"date rape drug†.Rohypnol’s sedative effects are about 7 to 10 times stronger than Va lium. Effects hit the victim within 15 to 20 minutes after being taken. Rohypnol is administered by young adults recreationally but most often and most commonly given to someone without them knowing they have just taken the drug. Due to its odorless and tasteless characteristics it can easily be slipped in a drink and dissolve quickly and therefore the person being administered the drug has no idea was has happened or that they have or are about to take it. The physical impact of rohypnol is it is known to cause temporary muscle relaxation, sleep, and impaired motor skills.Rohypnol also increases the effects of alcohol and other drugs without your knowledge. Also rophynol causes people to no remember certain things that went on such as events at a party or bar. Rohypnol also causes drowsiness, dizziness, loss of motor skills control, lack of coordination, blurred speech, confusion, all lasting up to and sometimes beyond 12 hours. The behavioral impact from use of rohypnol is depende nce for the drug and the withdrawal syndrome when the drug hasn’t been taken in a period of time. The most common effect from rohypnol is memory loss and the feeling of not being able to control yourself.Although rohypnol is most commonly used a sedative, chronic use of the drug can cause very aggressive behavior and possibly seizures. Also if a person has overdosed on rohypnol it is possible they can have problems breathing and fall into a coma. Long term effects from rophynol use are physical and psychological dependence. People who suffer from excessive use of the drug feel like they can’t live without it and are unable to do anything or feel good and have major withdrawal effects. The drug is most prevalent with teenagers and adults from ages 13 to 30.Due to its low cost only about $5 dollars per pill it makes the drug more available to people and then making it more abundant in the areas such as parties and bars. My assigned treatment plan is the Motivational Inte rference Therapy. What this treatment plan does is it offers counseling session’s motivational therapy and multiple step programs to help pull the patient out of addiction and back to living a healthy life. The first stage involves encouraging the patient to overcome their dependency behavior and tries to help them see the life without their addiction and try to convince them it is possible and build their confidence up slowly.The second step is to show the effects of their addiction on themselves and how they can remove it from their life just like it came in. This session is done 2 to 4 times depending on the patient’s addiction. The final step is to motivate the patient to completely give up on drugs and show them it is possible and that they can do it. The pros to this method are that they are consistently motivating, reminding, and showing you the positives to getting out of addiction.The cons to this method are that I think this style might not work for everyone and that some people may need a different type of program one that moves at a slower pace that allows them to really see themselves and what addiction has done to them. Also I think patients would need to spend more time at each stage to really break through their addiction. The drug addiction this method is made more is all cases of addiction but not so much the heavy addictions of drugs and alcohol but it can definitely help most cases of addictions.

In contrast to both Source 1 and 2 Norman Stone ( Source 3) does not blame Germany, instead he suggest that the tension in Europe for the last decade finally reached an ultimatum in the July crisis and the war was the fault of all Great powers. Mark Hewitson argument that German army and the government pushed for war before 1914 is reasonable and puts the blame on the German leaders, this view is also supported by ficher who said that Germany was making plans for annexation before the July crisis which suggest that Germany hoped to quickly win the war. It is also supported by the Council meeting in 1912 which held that it was Germanys consious decision to wage war for its own gain. This is of course talking about the Schlieffen Plan which is an agressive plan of sieging france to not fight a war on two fronts during the war. This contrasts with Mark ( source 1 ) who says that German leaders did not neccessary plan the war, Schlieffen Plan was created 1905, which suggest that German millitary leaders were thinking about planning the war way back before the tension in Europe was high. Norman ( source 3) does seem to disagree with source 1 as it says the situation in which the First World War did break out was forseen for five years beforehand this suggest that the politicians and leaders would of known about the possibilty of war, and therefore some planning would need to be done in case of the war. In conclusion the German Leaders alone cant be responsible for the outbreak of war due to tense situation in Europe, triple entente being put in place which would encircle and therfore endanger Germany. However the sources  suggest that to a large extent German leaders were contributing towards creating the tension in Europe by their dangerous policy, their army leaders who wanted to quickly start a war and go through Belgium to gain France which would cause Britain to enter the War with the hopes of Germany not having to fight on two fronts and also the contribution to making the relationship between Britain and Germany thorugh policies such as Weltpolitik or Flottenpolitik. Finally the situation in Germany nationalism and hysteria and also SDP meant that to bring the country together a war would be the only reasonable choice.

Her toys consist of a bike, baby dolls, Barbie dolls, tablet, computer, and PlayStation 4. These are the toys Janiya plays with everyday according to her mom. I was amazed at how smart she was when it came to thinking of a way to get things she wanted instead of asking for assistance anything she needs access to and is too high for her to reach she climbs on furniture to reach it or asks her mom or aunt to get it if she couldn’t according to her mom, Janiya is displaying her gross motor skills. She then came in the living room and proceeded to change the channel to Nick Jr. and Sponge Bob Square pants was on and Janiya started singing the theme song standing up in the chair dancing and singing showing off her cognitive and functional play skills trying to do what she sees on tv and also displaying her language development. Janiya’s mom then walked from her bed room in the living room explaining to Janiya that she has to help her with her homework  before she watches cartoons so she needs to go to her bedroom to retrieve her book bag Janiya is in the kindergarten. Janiya runs in her room to get her book bag and quickly came back into the living room with it displaying her gross motor skills again. Her mom pulls out her homework which consists of writing her first name 10 times and writing her last name 10 times she gives Janiya verbal cues on writing her name writing Janiya write and telling her which letter to write next she is encoding this information inside Janiya’s memory and she will eventually have this information in her long term memory and will be able to write her name without verbal cues in the future right now she is now processing information and developing her memory. Investigation of Sphincter Muscle Complex Abstract Background: The exact anus reconstruction is the critical in patients with imperforate anus  which is related to the correct diagnosis of sphincter complex. Objectives: The aim of this study  is exact investigation of the prineal region for ultrasound detection of place and pathway of  sphincter muscle complex. Patients and Methods: This descriptive cross-sectional study was performed at Mashhad  medical university during 2016. Transperineal sonography was done in ten patients (6-12 week  age, 8 male and 2 female) with imperforate anus. Results: The shortest distance between rectal pouch and skin was between 8 to 20 mm, but the  distance between rectal pouch and skin via the anal sphincter path was longer (11 to 23 mm). The multi-layer view of anal tubercle was seen in all patients except one. It had a curved and  occasionally parasagittal path and eccentric than muscle complex. Anal muscle sphincter  complex could be seen in all patients with 2- 3.6 mm, occasionally asymmetric. Conclusion: The multi-layer view of anal tubercle and the anal sphincter complex are the two  important sonographic findings, which can better differentiated the level of anal malformation  and act as an indicator for the location of pull through. Background Colon cancer is a Imperforate anus is one of the anorectal malformations (ARM) which is  characterized with abnormal termination of the hindgut. The anus reconstruction and fecal  continence is one of the critical aspects of treatment and surgery of these patients which is  related to the correct diagnosis of sphincter complex position and the anus reconstruction in the  appropriate place within the sphincter muscle complex . There are various surgical approaches and procedures for anus reconstruction, including the use  of preoperative MRI for diagnosis of type and level of anorectal malformation . In addition, there  are some articles about the use of MRI in the detection of sphincter complex and the anus path  guidance . However, later technique has some limitations and no available in all pediatric surgery  centers. In Imperforate anus (IA) patients, sonography is used to determine the level of disorder (low,  intermediate, high), which may be divided to three groups according to the distance between  perineal skin surface and rectal pouch. However, there isnt an exact cut off for their  differentiation and there is some diagnostic overlap . Some articles consider above 15mm as high  type and below 10 mm as low type ARM , although another numbers between 5- 25 mm are also  considered as cut off point in articles. The passage of rectum from levator ani muscle in  transverse view of infra-coccygeal plan is another method that can help in differentiation  between high and low groups. Furthermore, it is also used for diagnosis of internal fistula  (rectourethral, rectovaginal and etc) which can be helpful in determining level of disorder .   However in practice, these criteria have a little value for operation protocols and the surgical  planning is mostly according to clinical criteria and intraoperative findings.   The exact localization of anal tubercle and muscle sphincter complex with preoperative  sonography can help the surgeon to select the less invasive surgical technique and improve  surgical results. 2. Objectives The aims of this study is exact investigation of the prineal region for detection ultrasonic criteria  of place and pathway of muscle complex and anal sphincter in the children with Imperforate  anus. 3. Patients and Methods This descriptive cross-sectional study was performed at Dr. Sheikh pediatric Hospital during  2016 after being approved by the Institutional Review Board of Mashhad University of Medical  Sciences, Iran and with its grant. In this study, ten patients (6-12 week age) with imperforate anus were selected after obtaining  informed consent from their parents. Nine patients had previous colostomy within the two days  of life and in another remaining one with rectovestibular fistula didnt previously performed it.   Patients were also evaluated for associated congenital anomalies (VACTERL-H and etc.).   The children were placed in lithotomy position and Foley catheter was passed from distal limb of  colostomy to the rectum and the balloon was inflated and retracted backward and fixed. For  better visualization of rectal pouch, normal saline was injected through fixed Foley catheter in  rectum. After prep drape as the first step, transperineal sonography was done by an  experienced pediatric radiologist. Sterile gel and Betadine were used for sonographic window.   The ultrasound devices used in this study was sonosite Model S Nerve with a 12 MHz linear  superficial probe. The sonographic criteria such as shorten distance of rectal pouch from the surface of skin,  distance of rectal pouch from the surface of skin in anal sphincter complex pathway , the  presence of internal fistula, multi-layered view of anal tubercle, state of anal sphincter complex  were evaluated in both sagittal and coronal planes. The Distance between rectal pouch from the  surface of skin in the shortest path and this distance in correlation with sphincter complex were  separately measured. The distance between the rectal pouch and the surface of the skin and the presence of internal  fistula were evaluated in the sagittal sonographic plane. Internal fistulas can be identified by changing of the rout the echogenic mucus of the rectum  toward urethra or vagina. The multi-layered view of anal tubercle is exactly similar to gut signature, and visualized as a  peripheral hypoechoic layer with two central parallel echogenic lines just below the skin  (dermis). It has vertical position than to anal pit (Fig. 1A) and is visible only in coronal plan.   Anal sphincter complex is noticeable as circular muscular tissue in the depth of the subcutaneous  perineal area and is visible in coronal plan (Fig. 1B). 4. Results Table 1 showed the demographic and sonographic findings of ten imperforate anus patients with  were selected for this study. Eight patients were male and other two were female.   The shortest distance between the rectal pouch and the skin surface was between 8 to 20 mm, but  the distance between the rectal pouch and the surface of skin via the anal sphincter path was  longer and between 11 to 23 mm, that it was 3- 8 mm. (4.7 mm mean) longer.   There were rectourethral fistula in 7, rectovaginal fistula in 1, rectovestibular fistula in 1, and in  another one patient no fistula was detected. In patient with rectovestibular fistula, anal sphincter  complex was pushed back toward the coccygeal tip due to fecal material pressure.   The multi-layer view of anal tubercle was seen in all patients except one (rectovestibular fistula  patient). The maximum outside diameter was 3-4 mm, but in patient with cloacal anomaly, it had  about 10 mm in sagittal plan on the posterior of prineal orifice. In often patients, the anal  tubercle path until center of muscle complex had a curved and occasionally parasagittal path and  it wasnt straight (Fig. 1C). The visible length of multi-layer view of anal tubercle was 5-8 mm,  and it attaches to the mucus of muscle complex eccentric or concentric (Fig. 1D).   Anal muscle sphincter complex could be seen in all patients. The muscle complex thickness had  2- 3.6 mm which occasionally was asymmetric (Fig. 1E). 5. Discussion Imperforate anus is a congenital disease with abnormal termination of hindgut which have a wide  spectrum of muscle sphincter complex development (from near-normal muscles to complete  absence of the sphincter muscle). Routinely, depending on the level of the obstruction in above,  middle and below of muscle sphincter, this anomaly is categorized into three groups (High,  intermediate, low type) . The numerous factors especially the fecal continence after the surgery  related to the diagnosis of the exact place of sphincter muscle complex . There are many articles about the role of preoperative MRI in the determining of the type and  level of anorectal malformation which can be helpful in planning and the prediction of the  prognosis and also investigation of the spinal and urethral anomalies which indirectly effect on  the management of disease and operation . MRI has also a role in these patients for  demonstration of the status of sphincter muscle complex, the symmetry of the sphincter, the  perirectal fibrosis . The post-operative MRI is use to evaluate surgical results and the passage of  pulled-through bowel from the center of sphincter complex . Recently, there are some few  articles about the use of MRI in the localization of sphincter complex and the anus pathway as a  guidance instrument . The sonography is usually used to determine the level of disorder (low, intermediate, high)  indirectly basis on the distance between perineal skin surface to the rectal pouch and the internal  fistulae visualization. Although, this approach isnt very determinative and there are a lot of  diagnostic overlap in this field .   In review article, we find only one paper about the detection of the passage of rectum from  levator ani muscle in transverse view of infra-coccygeal plan that can be helpful for  differentiation of high and low groups . The pre-operative exact localization of anal tubercle and especially sphincter muscle complex  with sonography can be helpful for surgeons to select less invasive approaches that determines  the future fecal continence of patient. In this study, with exact ultrasound investigation of prineal region, we noticed two sonographic  findings which can be helpful in patients with imperforate anus to determine the proper path of  anal canal for pull-through operation. These findings were multi-layered view of anal tubercle  and sphincter muscle complex. Multi-layered view of anal tubercle is exactly similar to gut signature and was determined as a  peripheral hypoechoic layer with two central parallel echogenic lines. This view was probably  the result of fetal anal tubercle as a result non-ruptured anal membrane and non recanalized anal  canal. This view wasnt seen in patient with recto-vestibular fistula. Although it had 3-4 mm  diameter, in Cloacal anomaly patient, it had about 10 mm anterior-posterior diameters in sagittal  plan that probably due to fetal merge of anal and vaginal orifices. In most of patients anal tubercle wasnt straight and had parasagittal position and slightly  curvature with eccentric attachment to center of sphincter muscle complex. These can explain  pathophysiology of disease. Anal sphincter and muscle complex was seen as a circular muscular tissue bulk that surrounds  the echogenic mucus of gastrointestinal tract. It was visible on the coronal plane at depth of  subcutaneous fat of the perineal area with 2-3.6 mm thickness. This complex was visible in all of  our 10 patients. In a patient with rectovestibular fistula, this complex pushed backward to the near of coccyx  probably due to fecal retention. Although most patients with Imperforate anus and recto-perineal  fistula categorized as low type, but this patient had high type malformation because the sphincter  complex has been pushed backward and tract of fistula lie above of muscle complex. In this  patient, based on the distance of rectal pouch to the skin (9 mm) and based on clinical findings  alone and without attention to muscle sphincter, the probability of successful surgery was too  low without sonography guide. Then, visualization of the sphincter muscle complex and rectal  pouch is an important sonoghraphic findings in imperforate anus patients that can differentiate  better the patients to the high and low malformation.   In addition, the result of this study shows that the distance between rectal pouch and skin without  attention to muscle sphincter is unreliable and can make a serious pitfalls and unaware  complications. In all patients, the distance between rectal pouch and skin through anal tubercle  and muscle complex (11mm) was longer than shorten distance between rectal pouch and skin (3-8 mm mean: 4.7 mm). In lithotomy position, it is important to notice that multi-layered view of anal tubercle and anal  sphincter complex was only visible in coronal view and was invisible in routine sagittal and  transverse view, then it may be ignored and didnt notice to it in literatures. In the review of  articles, we did not encounter a similar publication about the use of this findings in patients with  imperforate anus, although there are many articles about the use of sonography to determine the  anal sphincter complex in adults in various diseases .   This is a preliminary cross sectional study with the low number of patients. In addition, the  frequency of the ultrasonic probe device were our study limitations. Exact examination of  perineal region with high-frequency probes (14 to 20 MHz) with high amount of the patients can  provide better and more reliable results. Conclusion: The multi-layer view of anal tubercle and the muscular bulk of anal sphincter complex are the  two important sonographic findings, which can better differentiated the level of anal  malformation and act as an indicator for the location of anal sphincter pull through in patients  with Imperforate anus.

Along with Enlightenment came European struggle with the monarchy. This led to ideas of a self-governed people and, along with the separation of individuals from religion and government, would inevitably influence Thomas Jefferson’s writing of The Declaration of Independence. The beliefs in equality, justice, and morals were outstanding ideas from the Enlightenment that moved Jefferson to write in the instructive manner in which his purpose was to lead a group of people to believe in â€Å"life, liberty, and the pursuit of happiness.† The Enlightenment influenced Jefferson’s writings and was responsible for his instructive writing style. Putting a Close Eye on LASIK Laser-Assisted In situ Keratomileusis (LASIK) is a surgical operation intended to allow an individual to live independent of glasses or contacts. Since 1995, when the Food and Drug Administration approved the type of laser used in corrective eye surgery, optometrists have developed a number of different procedures to clear up foggy or blurry vision in one's eye. In 1998, the Lasik surgery became the most common type of surgery, and remains the number one refractive eye surgery today. The surgery itself lasts no longer than 15 minutes per eye, and begins with the doctor placing a number of eye numbing drops into the patient's eyes to eliminate any possibilities of pain during the procedure. The first step in the procedure involves the cutting of a flap in the cornea by a computer programmed device called a microkeratome. By cutting the flap, the surgeon is able to use tiny tweezers-like instruments to unpeel the sliced flap. With the flap peeled back from the cornea the laser is used to remove small pieces of the inner part of the cornea, but the laser has only a specified wavelength which does not allow it to pass through the cornea to any other portion of the eye. The flap is then repositioned without stitches and is secure after seconds of drying ("Lasik Eye Surgery"). Doctors claim that the surgery is so popular because the patient's vision is restored rapidly and there is little to no post- surgery pain. Community vs. Individualism

Community vs. Individualism Individual and community are two words whose meanings contrast each other. An individual is one who is self-sufficient and not reliant on outside forces to get whatever task complete. A community however, is a group of individuals whose sufficiency is intertwined with one another, and therefore can rely on each other. In her narrative essay, â€Å"Community and Diversity†, Rebekah Nathan unveils the individualistic society that was her college experience. She describes life as a freshman from her own point of view and examines the characteristics of the student population at her university.She uses her own experience as a microcosm of university life throughout the country, but her account does not represent all schools appropriately. She says, â€Å"They (students) genuinely want to have a close community, while at the same time they resist the claims that community makes on their schedule and resources in the name of individualism, spontaneity, f reedom, and choice†(Nathan 233). Her description exposes the student body as being largely individualistic, which is not always the case.In this essay, I will argue that not all campus life in universities today is individualistic, because there is still a great deal of community amongst the student body. To begin with, saying that campus life is geared towards being an individual is not always true. Students go out of their way to join clubs and participate in study groups and review sessions. They want to come together and form an alliance for academics or just to be social. In her essay, Nathan claims that our nation as a whole is becoming an advocate of â€Å"individualism†.While this may be true, it in no way means that university life is affected by this. People come to college to not only further their academic quest, but to expand their horizons and to network with their peers. For example, an incoming freshman has the capability of joining a learning community related to their interests or what they think their future major might be. If a business major wants to get involved outside the classroom, there are a number of options available to them, such as an academic service learning program, or joining a business fraternity.The variety of choices one has is overwhelming, and these programs and clubs would not be available if they were underused, they would simply turn over. Equally important, is the fact that in today’s classrooms, group work is often either encouraged or required. Professors expect classmates to get along and collaborate (just not when their teaching), either during an assignment in class or for a project assigned to work on outside the classroom. How so then, as Nathan puts it, are students getting more and more individualistic? In his essay, James Surowiecki explains that there is power in the form of the small group.He says â€Å"small groups have the opportunity to be more than just the sum of their parts† (441). This means that a group can outperform even the smartest of its individuals on their own. That said it isn’t hard to see the benefit of community values in an academic workplace, rather than individualism. A good example of this is if you were to put five students in a group to complete a series of math equations. If it took the smartest individual thirty minutes to complete twelve problems, it would likely take much less time for the group, including that same individual, to finish the task.For this reason, privatizing the campus experience is not beneficial to the learning process, rather it is harmful. Integrating students from group work keeps them from building social skills that they will need in the real world after college, and it additionally gives students an absence of the cognitive diversity one could discover while working in small groups. Moreover, if an institution wants to inspire a common education goal amongst its students, there are several ways i t could initiate it.For one, the university could instill living learning communities into the student housing. This allows students with the same majors or interests to live in the same suites, as well as giving them the some of the same core classes. Consequently, students can share information about classes and even form study groups without even leaving their dorms.
